Dento-alveolar and oral and maxillofacial surgery in Targu Jiu

At Hevadent, our team includes specialists in dento-alveolar surgery and in oral and maxillofacial surgery, who diagnose and treat cases that call for complex surgical procedures in the mouth. These include extractions of impacted or partly impacted teeth, apicoectomies, surgical treatment of cysts and other procedures on the bone and soft tissues of the mouth. Each procedure is planned on the basis of the consultation and the imaging, which can be carried out at the clinic.

In short

At Hevadent, the specialists in dento-alveolar surgery and in oral and maxillofacial surgery carry out procedures such as extractions of impacted or partly impacted teeth, apicoectomies, surgical treatment of cysts, reshaping of the alveolar ridge, and procedures on the bone and soft tissues of the mouth. A specialist consultation costs 150 lei.

How complex a tooth extraction is gets settled after the consultation and the X-rays. When a specialist surgical procedure is needed, the dentist recommends an assessment by a specialist. Book a consultation to find out which treatment suits your case.

What dento-alveolar and oral and maxillofacial surgery cover

Dento-alveolar surgery and oral and maxillofacial surgery include procedures on the teeth, the jawbones and the soft tissues of the mouth. They take in extractions of impacted or partly impacted teeth, apicoectomies, removal of cysts, reshaping of the bone ridge, surgical exposure of an impacted tooth for orthodontic treatment, procedures on the bone before dental implants are placed, and procedures on the soft tissues of the mouth. Most of these procedures are carried out under local anaesthetic, at the practice, and the patient goes home the same day.

The difference from a routine extraction is not only one of time. For an impacted tooth, the surgeon may raise a flap of gum, may remove the bone covering the tooth and, in some cases, may section it to make the extraction easier and reduce the force on the surrounding bone. The work is done close to important anatomical structures: the inferior alveolar nerve, for lower wisdom teeth, and the maxillary sinus, for upper teeth. That is why these procedures may call for the experience of a specialist, and why their planning starts from the consultation and the imaging.

The prices in the list reflect how complex the procedures are. A partly impacted wisdom tooth costs 500 lei, a fully impacted one 650 lei, and an impacted canine or premolar 750 lei. An apicoectomy on front teeth and ridge reshaping on one arch cost 700 lei each, and a cystectomy 600 lei. An internal sinus lift costs 1,000 lei, an external one 2,500 lei, and bone grafting 1,000 lei per gram. The list closes with procedures on the soft tissues, many of them carried out with a laser.

The situations that reach the specialist

Some cases start with pain, others are found on an X-ray. Depending on the clinical and radiological assessment, the dentist may recommend a consultation with a specialist in dento-alveolar surgery or in oral and maxillofacial surgery.
  • A wisdom tooth still in the bone or only half through

With no room to come through, a wisdom tooth can stay covered by gum or bone, and bacteria can gather beneath the gum. The area can flare up periodically. Where extraction is indicated, it costs 500 lei for a partly impacted tooth and 650 lei for a fully impacted one.

  • A root-treated tooth still has infection at the tip

Sometimes, when retreating the root canal is not possible or not indicated, the answer may be an apicoectomy: the tip of the root is removed together with the affected tissue around it. On front teeth it costs 700 lei.

  • You have a tooth that never came through

A canine left in the bone can be taken out, at 750 lei, or, as part of orthodontic treatment, exposed surgically and drawn gradually onto the arch. Another option, which involves surgical exposure and bonding an orthodontic button, costs 550 lei.

  • The X-ray shows a possible cystic lesion in the bone

A cystic lesion can grow slowly, without pain, and may be found by chance on an X-ray taken for something else. Diagnosis needs a specialist assessment and, in some cases, histopathological confirmation. Where surgical removal is indicated, a cystectomy costs 600 lei.

  • You wear a denture and the ridge is sore

After multiple extractions, the bone can be left uneven and cause discomfort beneath a denture. Reshaping the ridge on one arch costs 700 lei and can be carried out before removable prosthetic work.

  • There is too little bone where a tooth is missing

Bone grafting can rebuild the bone volume needed to place a dental implant, and a sinus lift allows the bone height to be increased in certain areas of the upper jaw. An internal sinus lift costs 1,000 lei, an external one 2,500 lei, and bone grafting 1,000 lei per gram. Planning these procedures is part of implant treatment.

  • A growth on the gum or a frenum pulling at the lip

Certain growths on the lining of the mouth may need surgical removal, following a medical assessment. The procedure costs 350 lei by the conventional surgical method or 600 lei with a laser. Laser frenuloplasty costs 350 lei, and laser gingivoplasty 175 lei per tooth.

From consultation to healing

The steps below set out how a surgical procedure unfolds, from the first consultation to healing. How long recovery takes varies with the type and complexity of the procedure.
  1. The consultation and the imaging

A specialist consultation costs 150 lei. X-ray imaging is essential for planning surgical procedures. A panoramic X-ray costs 85 lei in digital format or 100 lei on film, and a CBCT scan, when needed, between 175 and 400 lei, depending on the area examined.

  2. Talking through medication and conditions

Say which medicines you take, especially anticoagulants, osteoporosis medication (including bisphosphonates) or diabetes treatment, and what conditions you have. This information can influence the preparation for the procedure and its timing; it is not simply a form to be filled in.

  3. Anaesthetic and the procedureusually 7–10 days before the stitches come out, if needed

The area is anaesthetised locally and the surgeon checks it has taken before starting. For an impacted tooth or a cyst, a flap of gum may need to be raised and part of the bone removed. At the end, if needed, stitches are placed, which either come out at a later visit or dissolve, depending on their type.

  4. The first hours afterwards

A blood clot forms over the wound, which protects the area and helps it heal. Bite firmly on the gauze for as long as you are told, hold something cold against the cheek as the surgeon advises, and do not rinse your mouth for the first 24 hours. Avoid alcohol, smoking and drinking through a straw, because they can affect how the clot forms and stays in place.

  5. Healing and what comes nextthe bone can take a few months to rebuild

The gum usually heals in the first weeks, while the bone takes several months to rebuild. After a sinus lift or bone grafting, the timing of implant placement is decided by the surgeon, according to how healing progresses and to the clinical and imaging assessment. In some cases the implant can be placed in the same procedure as the bone graft.

A wisdom tooth that keeps flaring up needs a medical assessment. Book a consultation to find out which treatment suits you.

How to prepare for the procedure

Eat before your appointment, if the procedure is under local anaesthetic: the area will be numb for a few hours afterwards, and on an empty stomach feeling unwell is more likely. Do not come after a sleepless night, and avoid alcohol before the procedure. If procedures make you anxious, bring someone with you and tell the surgeon before you start.

Bring the full list of your medicines, with doses. Anticoagulants and antiplatelet drugs are not stopped on your own initiative, because interrupting them can carry significant risks. Any change to your treatment is decided by the doctors involved. Osteoporosis medication, such as bisphosphonates, matters particularly for procedures on bone. Say as well if you have had surgery recently or are undergoing cancer treatment.

Plan the day after. Avoid heavy physical effort and demanding activities straight after the procedure, as the surgeon advises. If you work shifts, pick a time after which you can rest.

The clinic sits at Strada Victoriei nr. 170, across the road from Lidl, and there is private parking accessed from Strada Victoriei. The timetable is 08:30 to 19:30 Monday through Thursday, and 08:30 to 15:00 on Friday.

Worth knowing before a procedure

Why the price of a wisdom tooth varies so much

The list has three prices for removing a wisdom tooth, and the difference between them reflects the anatomy and the complexity of the procedure.

A fully erupted wisdom tooth costs 400 lei. A partly impacted one, only half through the gum, costs 500 lei, and a fully impacted one, still in the bone, costs 650 lei. Depending on where the tooth sits, the extraction may involve lifting the gum, removing part of the bone or sectioning the tooth so it comes out more easily.

How complex the extraction is gets settled after the consultation and the X-rays, which show the position of the tooth, its roots and its relationship to the neighbouring anatomy, such as the inferior alveolar nerve or the maxillary sinus.

When an apicoectomy is done instead of an extraction

A tooth with infection at the tip of the root is usually treated endodontically, that is, through the root canal. If the infection persists and root canal retreatment is not possible or not advisable, a surgical alternative can be an apicoectomy: the surgeon removes the root tip together with the affected tissue around it, so the tooth can be kept on the arch. On front teeth, an apicoectomy costs 700 lei.

There are also situations in which the tooth can no longer be kept, such as certain vertical root fractures or advanced loss of the supporting tissues. The surgeon decides, after the clinical and X-ray assessment, whether the tooth can be kept or whether it needs to come out.

What a dental cyst is and why it is removed

A dental cyst is an abnormal cavity, usually filled with fluid, that can form in the bone, often around the root tip of a tooth whose pulp has died or around the crown of an impacted tooth. It can grow slowly and often causes no symptoms. As it enlarges, it can lead to loss of the surrounding bone and shift the neighbouring teeth.

Surgical removal of the cyst, called a cystectomy, costs 600 lei. It is usually done under local anaesthetic, by opening surgical access and removing the lesion. The diagnosis may need histopathological confirmation. Depending on the case, the tooth involved may be kept with treatment or may need to be extracted, a decision taken after the clinical and X-ray assessment.

What a sinus lift and bone grafting mean

After a tooth is lost, the bone in that area can shrink, and at the back of the upper jaw the maxillary sinus can extend into the space where bone was lost. A sinus lift raises the sinus membrane to increase the bone height available for implants, sometimes with the help of a bone graft material. The internal version is done through the site prepared for the implant and costs 1,000 lei, the external one, through a window in the side of the bone, 2,500 lei.

Bone grafting rebuilds bone volume in width or height and costs 1,000 lei per gram of material. Both are surgical procedures linked to dental implant treatment, whose stages are described in the implant section.

Frequently asked questions

Does a surgical procedure hurt
The procedure is usually done under local anaesthetic, which prevents pain in the area being treated. The surgeon checks the anaesthetic has taken before starting. If you feel pain during the procedure, tell the surgeon, so the anaesthetic can be topped up. Once the anaesthetic wears off, pain and swelling can appear, which can be eased with the recommended medication and cold compresses.
What is the difference between a routine and a surgical extraction
In a routine extraction, the tooth can be removed with dedicated instruments, without additional surgical access. A surgical extraction may involve lifting the gum, removing part of the bone or sectioning the tooth, depending on its position and particular features. The type of procedure is settled after the consultation and the X-rays, and complex cases may need a specialist.
What does an impacted wisdom tooth extraction cost
Removing a partly impacted wisdom tooth costs 500 lei, and a fully impacted one 650 lei. Removing an impacted canine or premolar costs 750 lei. A specialist consultation costs 150 lei, and the X-ray imaging needed is charged separately.
Do all wisdom teeth have to come out
No. A fully erupted wisdom tooth, correctly positioned on the arch, healthy and that can be cleaned properly, does not need to come out if it causes no problems. Removal may be recommended when there are repeated infections, decay that cannot be treated properly, damage to the neighbouring tooth or other complications. The decision is made case by case, after the consultation and the X-rays, not preventively for everyone.
Why do I need a CBCT scan before the procedure
Unlike ordinary X-rays, a CBCT scan gives three-dimensional images of the teeth and bone, showing their relationship to the neighbouring anatomy, such as the inferior alveolar nerve or the maxillary sinus. For impacted teeth, cysts or procedures on bone, the surgeon may recommend a CBCT scan for a more detailed assessment and for planning the treatment. The scan can be taken at the Hevadent clinic, and the price starts at 175 lei for half an arch.
How long is recovery after a surgical procedure
Recovery depends on the type and complexity of the procedure. The gum usually heals in a few weeks, while the bone can take a few months. Swelling, where it appears, is usually most marked in the first 2–3 days, after which it starts to go down. If non-dissolving stitches were placed, they usually come out after 7–10 days. The surgeon will advise how long to avoid physical effort and explain what you need to do for a proper recovery.
I take anticoagulants or I have diabetes. Can the procedure be done
In most cases yes, but certain precautions are needed. Anticoagulants should not be stopped on your own initiative, and any change to your treatment is decided together with the doctors involved. Controlled diabetes does not usually rule the procedure out, but blood sugar levels can influence healing. Tell the surgeon about your conditions and all the medicines you take, and bring recent blood tests if you are asked for them.
I am afraid of the procedure. What can I do
Talk openly with the surgeon about your fears before the procedure. They can explain the stages of the treatment, roughly how long it takes and how the anaesthetic is given. If you have had unpleasant experiences with past treatment, tell the surgeon, so the approach can be adapted to your needs.
